QuestCares may help you save money with special program prices for eligible patients. By presenting your QuestCares card and your doctor's order at any participating Quest Diagnostics Patient Service Center you will:

• Receive special member prices on a group of the most frequently ordered lab tests performed by Quest Diagnostics;
• Be advised of the cost of your test(s) (if included in the program) before you provide your specimen;
• Avoid unexpected costs by paying at the time of service.

QuestCares offers eligible patients special program prices on many of the routine lab tests physicians order most frequently. These prices are only available on tests ordered by a physician or other authorized person to QuestCares members, and are only available for specimens collected at participating Quest Diagnostics Patient Service Centers.

Enrollment in the program is just $10 per year, and members have access to special prices on a group of the most frequently ordered lab tests. Only people without insurance can join. Each family member must obtain his or her own card.

Enroll in QuestCares today with two convenient options

1. Enroll at a Patient Service Center. This option activates your membership immediately. See the enrollment form in the Membership Brochure for more details.
2. Enroll by mail. Call 800-520-7010 to receive a membership application. Your participation begins when you receive a membership card in the mail (4 to 6 weeks).

Once enrolled, you'll receive a QuestCares welcome kit with:

• A complete list of lab tests included in the program with the program prices
• Your QuestCares membership card for use at participating Quest Diagnostics Patient Service Centers. (The card is valid for the calendar year in which the application and fee are submitted.)
