Terms and Conditions

  1. You must enroll in QuestCares™ and provide your membership information at the PSC (Patient Service Center) to receive program pricing. To enroll, contact 800-520-7010 for a membership application. Complete enrollment information and submit with enclosed payment.
  2. QuestCares is available at participating PSCs in certain geographic areas:
    • Colorado
    • Delaware
    • Montana
    • Southern New Jersey
    • New Mexico
    • Western Pennsylvania
    • Greater Houston area, Texas
    • Utah
    • Wyoming

  3. You can join QuestCares only if you are uninsured. If you have Medicare or other health insurance coverage that includes coverage for medical laboratory tests, you are considered insured.
  4. Your QuestCares card is valid for the calendar year in which you enroll. To continue your participation, membership must be renewed annually, including payment of annual fee for any succeeding year.
  5. Restrictions apply. Approximately 100 tests of Quest Diagnostics choice are available for purchase, with your doctor's order, through this program. From time to time, tests and the prices of the tests included in the program may change. Tests are performed at Quest Diagnostics only. Pricing does not apply to tests performed at other laboratories, hospitals, physician's offices, etc.
  6. Tests your doctor ordered that are not available through the program are typically available through Quest Diagnostics at patient list price. You must pay at the time of service for tests that are not included in the program.
  7. You must visit a participating Quest Diagnostics Patient Service Center to obtain these services (e.g., pay for your tests and provide your specimen). Program pricing is not available when the specimen is provided at any other location (e.g., doctor's office). A specimen collection fee is standardly charged at our PSCs; however, this fee is also discounted for QuestCares members.
  8. If you are under 18, a parental guardian must submit the enrollment form. There is no age restriction to join.
  9. One card per person. Each family member must obtain and pay the enrollment fee for his or her own card.
  10. This program is not insurance.
  11. Quest Diagnostics may change the program, its terms or eligibility requirements without prior notice. Quest Diagnostics may cease to offer the program at any time with 30 days notice to active members.
  12. The enrollment fee is non-refundable, even if the program is changed or canceled.
  13. By providing your contact information and checking the “okay to contact” box on the enrollment form, you consent to receive information about the program, lab testing and special promotional offers. You may ask to have your name removed from the mailing list by visiting QuestCares Contact Us. Be sure to specify that you wish to unsubscribe from the QuestCares program.
  14. QuestCares prices do not apply to any tests ordered or performed prior to program enrollment. Access to QuestCares prices requires that patients supply their QuestCares card at the time the specimen is taken at the Patient Service Center (PSC).
  15. Use of program card cannot be combined with any other offer.
